Case Manager/CPST

To support unsheltered individuals/families in preparing for, securing, and retaining permanent housing, by assessing barriers to permanent housing, developing goals, objectives, and plans that address the barriers, which includes obtaining and retaining employment and other income; to provide individual mental health services and monitor progress made toward achieving the goals, objectives and ISP’s; to complete file documentation and reports on results and outcomes.  Performs other duties of a similar nature as may be required.
This is a grant-funded position.
